Caregiver and Family Assessments

Do you feel overwhelmed when caring for your loved one with dementia?

Few things are as stressful as trying to find help. Do you need help planning and preparing for the future?  We can help! Talking about concerns and developing a plan can help family members meet the challenging needs of being a caregiver. Our trained professional staff are available to provide a range of consultation services to help you and the members of your family.

Care consultation is a service provided by trained professionals to the caregiver and family members which incorporates personalized assessment, service plan development, trouble-shooting and periodic updates.  It considers and incorporates patient and caregiver needs, resource management, continuity in care, and linkages to community services.  Care consultation includes education and support tailored to the individual caregiver and/or family members.  Family consultation a service provided by trained professionals which offers an opportunity to bring caregivers and family members together to discuss issues of common concern, strategies to address them, and the opportunity to learn together in a safe facilitated forum.

These services are available to caregivers in Nassau and Suffolk Counties to assist with meeting the needs of the person with Alzheimer’s disease and other dementias, and to provide supports and services to you throughout the process.

Our trained professionals are available anytime or place to assist you and your family in planning for the future, linking you to community resources, give you information about your loved ones condition, and refer you to other services that you may need.  In addition, they will be a support to you; no matter what stage of the disease your loved one is in.

Consultations are done in the caregivers home or at another location in the community that is convenient for you and your  family.  Our staff will also inform you of  the additional free services that our program has to offer.
