The Guiding an Improved Dementia Experience (GUIDE) Model: Coming July 2025
GUIDE (Guiding an Improved Dementia Experience) offers comprehensive, compassionate, and personalized support for individuals and families navigating the challenges of dementia.
Created by the Centers for Medicare & Medicaid Services (CMS), the GUIDE model is designed to enhance the quality of life for people living with dementia, reduce caregiver stress, and help individuals remain in the comfort of their homes.
Parker at Your Door, a community health program of the Parker Jewish Institute for Health Care and Rehabilitation, proudly provides the full range of services covered by GUIDE. These include thorough assessments, personalized care planning, respite care, and continuous care navigation. Families also have 24/7 access to a support line, connecting them with dementia specialists for guidance and assistance with any dementia-related concerns. Additionally, GUIDE participants benefit from community support groups, educational presentations, training sessions, and memory cafés.
By integrating expert care, education, and community resources, GUIDE empowers families to navigate the complexities of dementia with confidence and support.
With the GUIDE model, families can navigate dementia with confidence, ensuring their loved ones receive the support they need while cherishing meaningful moments together at home.
As part of its commitment to high-quality dementia care, Parker at Your Door, a trusted community health program of the Parker Jewish Institute for Health Care and Rehabilitation, is proud to offer the full range of GUIDE-covered services beginning July 2025.
Eligible GUIDE beneficiaries
Medicare and dually eligible beneficiaries who have an attested dementia diagnosis by a participating clinician are eligible if they meet additional criteria, including:
- Not receiving hospice services
- Living at home or in a community-dwelling
- Enrolled in Medicare Parts A & B or dually eligible for Medicare and Medicaid
- Not enrolled in Medicare Advantage or Special Needs Plans (SNPs)
- Not residing in a long-term nursing home
- Not receiving PACE services
