Case Management and Counseling

Trauma is widespread. It occurs as a result of violence, traumatic experiences. abuse, neglect, loss, disaster, war and other emotionally harmful experiences and that lasting adverse effects on a person’s functioning and mental, physical, social, emotional, or spiritual well-being. The effects of traumatic events place a heavy burden on individuals, their families and those that support them. Although many people who experience a traumatic event will go on with their lives without lasting negative effects, others will have more difficulty and experience traumatic stress reactions. Addressing trauma requires a multi-pronged, approach inclusive of education and awareness, early identification, and effective trauma-specific assessment and treatment. Research shows that with appropriate supports and intervention, people can overcome trauma.

Case Management

Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for services to meet an individual’s and family’s needs.  Case management serves as a means for achieving wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation.

Our Case Management Program can help you access the services and support you need and assists clients in connecting to resources that can considerably improve the quality of their lives.  Our Case Manager will identify your needs through a comprehensive assessment and serve as your advocate to connect you to with: support groups, education programs, wellness programs, counseling services, our Café Connection, as well as other community supports.

Our Case Manager will visit you at home, check-in by with you by telephone phone, video conference and work with you and your family or whoever else supports you.

Counseling Services

Our Mental Health Counselor, specializes in working with trauma in people with dementia know how to address these unique challenges and how to offer support and encouragement and to help people with trauma and their families. Working in collaboration with our case managers, our counselors help individuals, couples or families.  Helps to process and manage thoughts, feelings, and situations related to trauma. Through a thorough assessment of the client’s psychosocial triggers, strengths, and personal needs, we will help to develop plans that are properly targeted at the client’s needs. Services include: (1) assessing clients’ needs and support systems, (2) crises intervention, (3) helping clients develop coping mechanisms, and, (4) help clients to identify priority problems and creates on an action plan. This service may entail referral to necessary medical professionals.

These services are available in-person or virtually.
